AYA Liability Waiver

I am not aware of any injury, illness or other health related issues that would restrict or limit my child’s ability to play competitive sports.

I agree to assume all risks and am responsible for any and all expenses, including but not limited to any medical or hospital expenses, doctor bills or other expenses which could be the result of an injury that may occur as a result of my child’s involvement in competitive sports’ games, clinics, and or travel to/from activities.

I do hereby release the Athletic Youth Academy or anyone acting on its behalf, its Coaches, administrators, officers, and agents from all liability including claims and suits in law or equity for any injury, fatal or otherwise.

I have carefully read this agreement and fully understand its contents, I am aware that his is a release of liability, hold harmless agreement and assumption of risk agreement and that it is a legally binding contract between the AYA Inc. and myself. I further understand that Athletic Youth Academy retains the right to use, for publicity and advertising purposes, photographs of participants taken at any event. I understand that this release is binding on my heirs or anyone making a claim. I sign of my own free will.
